2009 ADHS Licensing Empower Pack Information and Registration Form

General Fund appropriations for the Division of Licensing Services were decreased and the Department was granted the authority, in Laws 2009, Third Special Session, Chapter 10, to increase fees to replace the general fund appropriations. The Department has changed the fees in the new exempt rules to generate the funding necessary to replace the general fund monies for the health care and child care licensing programs. The new fees are shown below.
