Children in kindergarten through grade 12 are required to have vaccines against:
- Diphtheria
- Tetanus
- Pertussis (whooping cough)
- Polio
- Measles
- Mumps
- Rubella (German measles)
- Hepatitis B
- Varicella (chickenpox)
Adolescents who are at least 11 years old are required to have the above vaccines and two additional vaccines:
- A booster dose against tetanus, diphtheria, and pertussis from the Tdap vaccine.
- A quadrivalent meningococcal vaccine (MenACWY or MCV4) that protects against a bacterium called neisseria meningitidis that can cause meningitis, gangrene, and rapid death.

There are additional vaccines recommended by the Centers for Disease Control and Prevention (CDC) to keep school-aged children healthy that are not required for school entry. Please see your healthcare provider and CDC Immunization Schedules.

Obtaining copies of immunization records
Parents who need to obtain a copy of their child's immunization records should contact their provider's office or their local County Health Department. If they are unable to obtain a copy of the record from these locations, parents may also be able to obtain copies from the Arizona Department of Health Services, Immunization Program Office located at 150 N. 18th Avenue, Suite 120 Phoenix, AZ 85007. Parents should fill out the Immunization Record Request Form and fax or email the form along with proper identification to fax 602-364-3285 or email ASIISHelpDesk@azdhs.gov and identify where they would like the immunization record to be sent.
Records for children under the age of 18 can be sent to a school, day care or child care center, or a doctor’s or health care provider's office.
